(e.g., appointment reminders, alerts as your health status changes). Receive announcements (e.g., vaccinations, workshops, health alerts). Receive personalized medical education for your health. This includes information on diagnosis and treatment. Track progress of existing conditions with data sent to your physician and systematically monitored to alert your doctor. All of this information is automatically transferred to your physician electronically via a secure HIPAA compliant web portal. IMPORTANT NOTICE This app cannot be utilized unless your doctor is part of the Connect4Life community.
